Accident Waiver and Release of Liability
Lumos Selfie Studio, LLC is not responsible for any injuries inflicted upon any participating parties. Client(s) will be responsible for their children and for themselves and release Lumos Selfie Studio, LLC, employees and/or volunteers, property owners, building owners and anyone associated with ownership and/or management of location of photography and/or videography, from any claims against their person or their business.
I HEREBY ASSUME ALL THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that I have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity.
I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.
In consideration of my application and permitting me to participate in this activity, I hereby act for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Lumos Selfie Studio, LLC, employees and/or volunteers, representatives, property owners, building owners and anyone associated with ownership and/or management of location of photography and/or videography.
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this, and the above paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.
I acknowledge that Lumos Selfie Studio, LLC, employees and/or volunteers, property owners, building owners and anyone associated with ownership and/or management of location of photography and/or videography are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.
I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders and organizers.
The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
Equipment & Space Damage Waiver
This Liability Agreement ("Agreement") is made between Lumos Selfie Studio, LLC, located at 1941 1st Ave S, Seattle WA 98134 ("Studio"), and any individual ("Client") who books and utilizes the studio space.
1. Assumption of Responsibility
The Client agrees to assume full financial responsibility for any damage, loss, or destruction of Studio property, including but not limited to cameras, lighting equipment, furniture, backdrops, and rented gear caused by the Client, their guests, or hired personnel during their session.
2. Use of Studio Space
The Client agrees to use the studio space and equipment responsibly and in accordance with Studio rules. Any negligence, reckless behavior, or misuse leading to damage will be the sole responsibility of the Client.
3. Compensation for Damages
In the event of damages, the Client agrees to compensate the Studio for the full cost of repairs or replacements within one month of the damage occurrence.
4. Indemnification
The Client agrees to indemnify and hold harmless the Studio, its owners, employees, and affiliates from any claims, lawsuits, or liabilities arising from their use of the space.
5. Acknowledgment and Agreement
By checking the box, the Client acknowledges and agrees to all terms outlined above.
